Filler Fatigue: When Injectables Stop Being the Answer

In Los Angeles and San Diego, filler appointments sit on calendars next to haircuts. The American Society of Plastic Surgeons counted 5.3 million hyaluronic acid filler treatments nationally in 2024 and nearly 9.9 million neuromodulator injections such as Botox.

The appeal is easy to understand. Filler is quick, the result is immediate, and it wears off if you change your mind. For a lot of patients that last point turns into the problem.

How filler fatigue starts

Most hyaluronic acid fillers last between six months and two years, according to FACES+, the San Diego practice founded by plastic surgeon Dr. Steven R. Cohen. Longer-lasting options exist. Bellafill can last five years or more.

Patients who start in their late twenties often top up every year for a decade. As the face keeps aging, each appointment adds a little more to hold the same look. Over time the face can begin to look heavy or puffy, above all in the mid-face and around the eyes. Friends notice something without knowing what. Photos show it before the mirror does.

The underlying cause often goes unaddressed. Filler replaces volume, but it cannot lift tissue that has descended or tighten skin that has loosened. When sagging is the real issue, adding volume only makes the face larger.

Signs you may have reached the limit

  • You need more syringes each year to get the same effect.
  • Your cheeks look full but your jawline still looks soft.
  • The area under your eyes looks puffy after treatment.
  • Your injector suggests you “let things settle” before the next round.

None of these mean you did anything wrong. They suggest the tool no longer fits the problem.

What comes next

A structured injectable plan. Before moving to surgery, some patients benefit from a reset. A skilled injector may dissolve older hyaluronic acid filler, wait, and rebuild with less product in better-chosen places. FACES+’s nurse injectors have worked with Cohen for more than 17 years, and the practice offers a range from JuvĂ©derm and Restylane to Sculptra, which stimulates collagen over months instead of adding bulk.

Fat grafting. Your own fat can restore volume for far longer than filler because the tissue that survives becomes permanent. FACES+ uses a regenerative fat grafting technique that filters fat into grades for different layers of the face. The practice reports that the method retains about 80 percent of added volume at two years in patients under 55.

A lift. When descent is the main issue, surgery addresses it head on. FACES+ tells facelift patients to expect results lasting ten years or more. Cohen’s deep-plane approach repositions the tissue under the skin and pairs it with fat grafting to restore lost volume in the same operation.

Doing the math

Annual filler in several areas of the face adds up. Over ten years, many patients spend a sum comparable to a surgical procedure, and at the end they own nothing that lasts. That comparison does not make surgery the right choice for everyone. It does make the decision worth revisiting every few years rather than defaulting to the next appointment.

The Southern California look is changing

The overfilled face that defined a certain era of Los Angeles aesthetics looks dated now. Plenty of patients who once chased volume would settle for looking rested. FACES+ calls its facial surgery style the California facelift: natural, confident and at ease, with no pulled or overfilled features.

Talk to someone who offers both

A clinic that only sells injectables has little reason to tell you to stop. A clinic that only performs surgery has little reason to suggest you wait. Look for a practice that offers the full range, and ask what they would recommend for your face over the next ten years.
